Come join us for Lamprey Health Care's Annual 5K Road Race on Saturday, August 14, 2010 at 8:30am - walkers & strollers welcome!

The 5K begins at the Newmarket Community Center and provides a flat, fast course for beginners, intermediate and advanced runners! The Lamprey Health Care 5K offers everyone a great day, with refreshments & music, indoor restrooms, prizes for top runners, and a major raffle drawing! We have technical shirts for the first 100 runners/walkers registered, and for all kids under 12 we have a Kids Fun Run as well!

Our Annual 5K Road Race benefits Lamprey Health Care's Reach Out & Read Program by buying age-appropriate books for our pediatric patients, ages 6 months to 5 years. Reach Out & Read is a national program that focuses on early literacy by promoting reading and a lifetime love of books among children.

Parking is NOT available at the Newmarket Community Center, so please park at the Newmarket High School or Lamprey Health Care and enjoy the short walk to the Community Center.

Waiver and Release - Please read and sign below
The undersigned, in consideration for LAMPREY HEALTH CARE allowing my participation in the LHC 5K Road Race, acknowledge that my participation in the LHC 5K Road Race entails inherent risks such as death, injury and damage to property. Those risks include, but are not limited to, death, injuries and damages resulting from the following: 1. The negligence of LAMPREY HEALTH CARE, its owners, employees, representatives, volunteers or agents; the negligence of guests, visitors or persons who may be present at or participating in the LHC 5K Road Race; or the negligence of any applicable government entities; 2. Skips, trips, falls, crashes or other such accidents that occur while participating in the LHC 5K Road Race; or which may be caused by other persons' participation in the LHC 5K Road Race; 3.The negligence or lack of adequate training of LHC 5K Road Race employees, representatives, volunteers, or agents of LAMPREY HEALTH CARE who seek to assist with medical or other help either before or after injuries have occurred. I agree to release from all liability, discharge and promise not to take legal action against: (1) LAMPREY HEALTH CARE, its directors, owners, employees, representatives, volunteers or agents, (2) any guest, visitor or person present or participating in the LHC 5K Road Race, (3) any sponsor of the LHC 5K Road Race, their directors, owners, employees, representatives, volunteers or agents, or (4) any applicable governmental entities, their directors, employees, representatives, volunteers or agents. I agree to release the aforementioned persons from any liability to me, my heirs, next of kin, assigns or personal representatives for any losses, damages, claims or demand arising out of my death, injuries or damages to property, even if their individual or collective negligence contributes to such death, injuries or damages. Assumption of Risk: I certify to LAMPREY HEALTH CARE that I am eighteen (18) years of age or older, physically fit, have sufficiently trained for participation in the LHC 5K Road Race and have not been advised against participating in the LHC 5K Road Race by a qualified health professional. I freely and voluntarily assume complete personal responsibility for all risks and for my death or any injury or damage that may occur to me or my property as a result of these risks, even if such death, injury or damage occurs in a manner that is not foreseeable to me at this time. I realize that by voluntarily assuming the risks involved, I will be solely responsible for my death or any injury or damage that I sustain. I have read this Assumption of Risk thoroughly and understand the terms. My participation in the LHC 5K Road Race and my execution of this Assumption of Risk are both purely voluntary and I elect to do so in spite of the risks. Moreover, I understand that LAMPREY HEALTH CARE, and/or those authorized by LAMPREY HEALTH CARE, will be taking photographs of the LHC 5K Road Race and its related events. I hereby irrevocably consent to and grant LAMPREY HEALTH CARE and /or anyone authorized by LAMPREY HEALTH CARE, the exclusive right to the ownership and use of any and all photographs taken by LAMPREY HEALTH CARE, and/or those authorized by LAMPREY HEALTH CARE, that contain my person, image or likeness, for any lawful purpose whatsoever in connection with the LHC 5K Road Race and its related events.
